In April of 2019, Oneworld Accuracy in partnership with the pSMILE (Patient Safety Monitoring in International Laboratories) program of Johns Hopkins University, launched the Root Cause and Corrective Action (RCCA) Form for select members. The RCCA form aims to help the laboratories to identify and correct issues affecting the proficiency of test results and prevent any reoccurrence.
Despite precautions and training, issues can still happen, ISO as well as other standards require us to have efficient and effective root cause and corrective action processes. Just as an improper medical diagnosis leads to improper treatment, an improper root cause investigation provokes the wrong corrective action plan.
The Root Cause and Corrective Action (RCCA) Form is divided in five parts in order to dissect the investigation to not miss any details when identifying the root cause and apply the proper corrective action:

Strong root cause investigations and effectiveness checks are essential in building the proper corrective action system and ensuring that action plans truly solve the problem. Once a successful plan is found, standardizing it and sustaining it are essentials. It is important to make the corrective action part of the everyday process, so the incident does not repeat itself.
